INSIGHT: model and data sources for epidemiological models
If you are interested in a slightly more complex model for how the disease progresses through an individual, to use either for a "mass action" model or an individual-level "agent-based" model, the workhorse in the literature for the past few years has been work by Legrand et al 2007: http://www.ncbi.nlm.nih.gov/pmc/articles/PMC2870608/. This model contains separate compartments for different channels of infection (general community, hospital, funeral). Still pretty straightforwards.

For those looking for parameter values from individual-level data (as opposed to finding them by curve-fitting), the largest existing source of data is this article in the NEJM: http://www.nejm.org/doi/full/10.1056/NEJMoa1411100
